I am having the same issue from the Middle East getting to Slingbox located in Atlanta, GA via StrongVPN. I have changed the VPN location to a different US IP address and it didn't help. I have been using Visual Route Lite (free download http://www.visualroute.com/download.html) to trace slow IP addresses. I still cannot get this resolved and am running out of ideas.
We've had this system running for several months with no issues. Upgraded the Slingbox HD Pro v2.1.80 and SlingPlayer v2.04.521 about a month ago and no issues with the upgrade.
A week ago the bit rate was as high as 1700Kbps and the past two days it is 25-115 Kbps. Video very choppy and audio skips a lot.
