If it works sometimes then the code must be correct. Maybe it is only seeing a weak IR signal. Are you using the external IR Blaster ? If you are try moving it slightly, if you aren't then try using it,
Maybe the cable box goes into a 'deep sleep' after being in standby for some time ? Do you have to do anything differently with the real remote ?
